A. Definition : an implant-supported prosthesis that is removable by the patient. B. Consideration 1. Overdenture design with an overdenture bar retained by screws. B. The overdenture has a metal frame work incorporated into it that fits precisely over the bar. The removable overdenture is retained by clips,attachments, or fasteners.
Examples
1. Patient I A. Five osseointegrated implants have been placed in the maxillary completely edentulous arch and a cast gold bar connects all the implants. B. A maxillary overdenture has been fabricated over with three retentive clips and an open palate. This prosthesis is totally implant supported. C. The patient with maxillary overdenture in place shows the flange, which is useful for altering profile. This flange also stops any air escape that may adversely affect phonetics. The removable aspect of this prosthesis makes it easier for the patient to practice daily oral hygiene. 2. Patient II A. A completely edentulous maxilla with five osseointegrated implants. A cast gold bar connects the implants with Hader bar at the midline, and with two ERA attachments cantilevered distally. B. The maxillary overdenture with an anterior retentive clip and two ERA attachments. This prosthesis is joint implant and mucosal supported 3. Patient III A. A maxillary fixed prosthesis supported by six osseointegrated implants. This is a cast metal framework with denture acrylic and denture teeth. B. The maxillary fixed detachable prosthesis is supported by six implants. All implants had angulated abutments in place. Note that the width of the maxillary ridge is narrower than where the prosthetic teeth needed to be placed for esthetics and occlusion. This is a result of the resorption of the buccal of the maxillary ridge. 4. Patient IV A . Seven implants were placed in the edentulous maxilla. B. The anterior smile view shows that the acrylic margin and the metal components are not visible in the high smile of the patient.
